Homelessness Navigator pilot in Fredericton supports safer transitions from hospital

A new pilot program aimed at helping unhoused people transition safely back into the community after admissions at the Dr. Everett Chalmers Regional Hospital in Fredericton, New Brunswick is yielding positive results, the Horizon Health Network reports. The pilot program is modelled after MAP’s Navigator program at St. Michael’s Hospital in Toronto, whose staff have visited the Fredericton facility and consulted on the local navigator project.
