Ontario Community Health Profiles Partnership (OCHPP)

Data and maps to help match health-related services to community needs

The Ontario Community Health Profiles Partnership (OCHPP) examines socio-demographic and health outcomes data at the neighbourhood/small-area level and ensures the data is available to everyone. We do this through our open-access website and free health maps as well as partnerships with health and community-based organizations. Local level data and maps shine a light on health equity concerns that may be masked at larger areas allowing policy makers to identify and address gaps in access to care.

The free data, maps and tables at www.OntarioHealthProfiles.ca can help you explore and compare different communities’ health issues, use of health-care services such as primary-care attachment, demographics and location of health-related services and resources. As of 2026, OCHPP provides data for 700 neighbourhoods and local areas across Ontario. This represents 77%  of Ontario’s population.

We have outstanding partnerships and a staff of expert data and geo-spatial analysts. Most of our health data comes from population-based health datasets, de-identified patient records, and clinical and administrative databases housed at ICES (an independent, not-for-profit research institute funded by the Ontario Ministry of Health.)

OCHPP provides guidance and collaborates on linking “person & place”, “disentangling” geographies, and small area development:

  • We partner with organizations to help them better understand and predict trends in their catchment areas such as the movement of people or rates of disease stratified by income, gender and age
  • Identifying areas where the distribution of services may not match needs 
  • Calculating distances and travel times for patients to services
  • Contextualizing data and creating opportunities for new insights or avenues of exploration
  • Creating new neighbourhoods or local areas to identify gaps in care or where a need for health services exists
